Intheolddays,childrenwerefamiliarwithbirthanddeathaspartoflife. ThisisperhapsthefirstgenerationofAmericanyoungsterswhohaveneverbeenclosebyduringthebirthofababyandhaveneverexperiencedthedeathofafamilymember.
Nowadayswhenpeoplegrowold, weoftensendthemtonursinghomes. Whentheygetsick,wetransferthemtoahospital, wherechildrenareforbiddentovisitterminallyillpatientsevenwhenthosepatientsaretheirparents. Thisdeprivesthedyingpatientofsignificantfamilymembersduringthelastfewdaysofhislifeanditdeprivesthechildrenofanexperienceofdeath, whichisanimportantlearningexperience.
SomeofmycolleaguesandIonceinterviewedandfollowedapproximately 500 terminallyillpatientsinordertofindoutwhattheycouldteachusandhowwecouldbeofmorebenefit,notjusttothembuttothemembersoftheirfamiliesaswell. Weweremostimpressedbythefactthateventhosepatientswhowerenottoldoftheirseriousillnesswerequiteawareofitspotentialoutcome.
Itisimportantforfamilymembers, anddoctorsandnursestounderstandthesepatientscommunicationsinordertotrulyunderstandtheirneeds,fears,andfantasies . Mostofourpatientswelcomedanotherhumanbeingwithwhomtheycouldtalkopenly, honestly,andfranklyabouttheirtrouble. Manyofthemsharedwithustheirtremendousneedtobeinformed, tobekeptup-to-dateontheirmedicalcondition,andtobetoldwhentheendwasnear. Wefoundoutthatpatientswhohadbeendealtwithopenlyandfranklywerebetterabletocopewiththeapproachofdeathandfinallytoreachatruestageofacceptancepriortodeath.
